What Is Aged and Disabled Waiver Case MANAGEMENT Program?<\/h4>\n

The Aged and Disabled Medicaid Waiver (ADW) Program is a long-term care option that provides in-home care and community help. These services are an alternative to institutionalization.<\/p>\n

Case Managers are professionals who assess and identify your needs, help you plan for your care, and advocate for  and connect you with services and community resources.   These services are coordinated with the personal attendant or resource consultant agencies.<\/p>\n<\/div>\n<\/div>\n<\/div>\n
